Diagnostic algorithm for circadian rhythm sleep disorders (CRSD)

Abstract

The article provides guidelines for clinical diagnosis and treatment of circadian rhythm sleep disorders (CRSD). CRSD are a cluster of sleep disorders characterized by a de-synchronization between an individual's biological clock and the environmental 24h schedule. There are six main types of CRSD, namely: Advanced Sleep Phase Type (ASPT), Delayed Sleep Phase Type (DSPT), Irregular Sleep-Wake Rhythm (ISWR), Free Running Disorder (FRD), Shift Work Type (SWD), and Jet Lag Disorder (JLD). Physiological data and genetic studies in patients with CRSDs suggest that these disorders result from abnormal functioning of the circadian rhythm system. Difficulties in daytime functioning are one of the prominent characteristics of CRSDs. Individuals with CRSDs frequently fail to adjust to the normal accepted hours of activity The diagnosis of CRSD is based on clinical interview, sleep log diaries and/or actigraphic monitoring under free condition schedule. Melatonin administration and bright-light therapy have proved to be the most effective treatments for treating CRSD.
