Abstract
The chapter provides a comprehensive exploration of the cost-effectiveness evaluation of wound dressings, with a particular focus on chronic wounds such as diabetic foot ulcers, venous leg ulcers, and pressure ulcers/injuries. These conditions pose a significant burden on healthcare systems due to their prolonged healing times and high complication rates. The chapter introduces Markov modeling as a robust framework for evaluating the long-term costs and clinical outcomes associated with wound dressings. By simulating patient transitions between various health states such as unhealed, infected, healed, or death, this method allows healthcare providers to assess the direct and indirect costs of different treatment options and their expected distributions, depending on the characteristics of the relevant patient populations such as their wound infection or other complication risks. Additionally, the chapter highlights integration of quality-adjusted life years into cost-effectiveness models, facilitating holistic evaluations of both the clinical efficacy and patient quality of life. Monte Carlo simulations are introduced, and their utility is exemplified in addressing uncertainties in patient outcomes and treatment costs. These simulations provide probabilistic distributions that offer insights into the financial risks associated with wound care strategies. Several hypothetical but practical examples are discussed to demonstrate how more expensive wound dressings can prove to be cost-effective in the long run, for example, by reducing infection risks and minimizing clinical labor costs. This chapter not only emphasizes the importance of considering broader economic factors in wound care but also explores relevant modeling features for simulating real-world scenarios demonstrating usefulness for decision-making.
